Block Party Permit:
The Block Party Permit can be filled out in person at the 19th Ward office or the online submission form at https://www.19thwardmobile.com/block-party-permit/. Your permit will be emailed to you. The Fire Department will be requested after the permit is accepted by CDOT . You will not receive confirmation of their attendance. This is a first come first serve basis and depends on their work load for the day.
NOTE: In accordance with CDC recommendations, and City and State guidance, to avoid non-essential gatherings and any gatherings larger than 10 individuals, CDOT is issuing the following directive: No Block Party Permits will be issued for dates through July 15, 2020. CDOT will begin accepting Block Party Permit applications on July 1, 2020 for dates after July 15, 2020.
NOTE: The City’s 2020 Jumping Jack Program (scheduled to run from Saturday, May 23 through Sunday, September 6, 2020) has officially been canceled due to the COVID-19 crisis.
City of Chicago Senior Citizen Sewer Service Charge Exemption Form:

75% of the households involved with a petition must sign the petition.
When 75% of the households involved have signed the petition please bring to either 19th Ward Office for processing.
